5/6th Grade Boys Basketball Schedule

ALL 7th grade and older athletes must have a sports physical on file (valid for 3 years) in our office, before they can register for a sport.

Sign up for "Notify me" on the FMS Warrior Calendar to receive info regarding changes to the game schedules, bus time departures/arrivals, etc.

  • THINGS TO REMEMBER:

    • Note the start date of your activity/sport -- no reminders will be sent!
    • Students in 7th grade and up are required to have a sports physical on file in our office PRIOR to registering. Physicals are valid for 3 years. Get yours in early!  We just need a copy - you can click on our picture on the right to get our email address. The system will NOT LET YOU REGISTER without one on file!
    • Some sports have a limited number of spots available and will fill up fast so don't wait!
    • We encourage all families to complete the Educational Benefits form (formerly Free and Reduced Lunch application), as our price structure for sports is based off of your free/reduced status.
    • Class sizes are limited, register early!
    • First time users of the new system will need to create a SmartSchoolK12 account. You must have the Student ID number and birth date of one of your children to link your account to all of the members of your family. You can get student ID numbers from your Skyward Family Access Account. If you used the system last year, your login credentials will remain the same.
    • Fee reductions are based on a family’s qualification for the Federal Free/Reduced meal program. After you apply and have been approved, the registration system “knows” your F/R status and if appropriate, your fee will be reduced automatically when you check out. Families with extenuating financial circumstances can contact their respective activities office for assistance. If you would like to apply for the Free/Reduced meal program, please fill out the attached 2024-2025 Application for Educational Benefits.
